How These Tests Work
- A screening, not a diagnosis. A score shows how much a symptom is getting in your way. Only a licensed provider diagnoses.
- Private by design. Your phone does the scoring. We don't save your answers unless you choose to share them.
- Built for adults. These tests are for people 18 and older.
- Scores change. Life changes week to week. Take a test again in 2 weeks to see the trend.
After your score, you choose what happens next: get matched with a provider, email the results to yourself, or close the page. No account and no sign-up.
Why a Free Test Matters in Nevada
Nevada ranked 51st, dead last, in Mental Health America's 2025 State of Mental Health in America report. The state has about one mental health professional for every 400 residents. Long wait lists mean many people never get a first answer about what they're feeling.
A free test won't replace care. A free test gives you a place to start, and words to use when you talk with a provider.

Is an online test a diagnosis?
No. A screening test shows how much symptoms are affecting you right now. Only a licensed provider diagnoses depression, anxiety, or any other condition.
Is my information private?
Yes. Your phone scores the test. We don't save your answers unless you choose to attach your score to a request, email the results to yourself, or share your score anonymously.
Who created these tests?
The PHQ-9 and GAD-7 were developed by doctors and researchers and are used in clinics and doctor's offices across the country.
What should I do if my score is high?
Talk with a provider soon. Tell us what you need, and we'll email you within 24 hours with providers who have openings. If you have thoughts of ending your life, call or text 988 right away.
Can teens take these tests?
These tests are built for adults 18 and older. Teens are welcome to talk with a parent or a school counselor, or call or text 988. A parent or guardian is welcome to ask us for help finding a provider. Parents are welcome to use our parent checklist for teens.
How often should I take a test?
Every 2 weeks works well, especially if you're starting therapy or medication. Watching your score over time shows whether things are improving.
