What Is Spravato?
Spravato is the brand name for esketamine, a nasal spray made by Johnson & Johnson. Esketamine is a close chemical relative of ketamine. Instead of working on serotonin like most antidepressants, Spravato acts on glutamate, a different brain messenger involved in mood.
The FDA approved Spravato in 2019 for treatment-resistant depression in adults. In 2020, approval expanded to adults with major depression and suicidal thoughts or actions, alongside an oral antidepressant. In January 2025, the FDA approved Spravato on its own, without a daily antidepressant, for treatment-resistant depression.

Three things set Spravato apart from other depression medications:
- You never take Spravato home. Every dose happens at a certified office, under a clinician's watch.
- You stay for at least 2 hours after each dose so staff check your blood pressure and how you feel.
- Spravato is a Schedule III controlled substance, so every certified center follows a federal safety program called the Spravato REMS.

Only a psychiatrist, psychiatric nurse practitioner, or another prescriber decides whether Spravato fits you. Bring a list of every antidepressant you've taken, the dose, and how long you took each one. The list speeds up insurance approval.
Who Should Not Take Spravato
The FDA lists four situations where Spravato is never used:
- An aneurysm, or a history of aneurysm, in the brain, chest, belly, arms, or legs
- An arteriovenous malformation (a tangle of blood vessels)
- A past bleed in the brain
- An allergy to esketamine or ketamine
Talk through these with your prescriber before you start:
- High blood pressure, heart disease, or a past stroke or heart attack
- Pregnancy, breastfeeding, or plans to get pregnant. Spravato carries risk to a developing baby.
- A history of substance use, including alcohol
- A history of psychosis
- Age 65 or older. Studies in this age group didn't show a clear benefit for treatment-resistant depression.
- Other medications, especially benzodiazepines, opioids, stimulants, and MAOIs
What a Spravato Visit Looks Like
Plan on about 2 and a half to 3 hours from check-in to checkout.
- 1.Before you arrive. Skip food for 2 hours and liquids for 30 minutes before your dose, since nausea is common. In a Las Vegas summer, drink plenty of water earlier in the day so you don't show up dehydrated.
- 2.Check-in. Staff take your blood pressure. If your numbers run high, they wait and recheck, or reschedule.
- 3.Your dose. You give yourself the spray, one device at a time, 5 minutes apart. Each device holds 28 mg. A 56 mg dose uses 2 devices. An 84 mg dose uses 3.
- 4.Rest and watch. You settle into a recliner. Many people feel floaty, dreamy, sleepy, or disconnected for a while. Staff check on you and recheck your blood pressure about 40 minutes after the dose.
- 5.Checkout. After at least 2 hours, a clinician confirms you're steady enough to leave.
- 6.Getting home. Don't drive yourself. Someone else drives you home, or you take a rideshare. No driving or running machinery until the next day after a full night's sleep.
Bring headphones, a playlist or podcast, an eye mask, and a light sweater. Medical offices in Las Vegas keep the air conditioning cold.
How Often You Go: The Spravato Schedule
For treatment-resistant depression, the FDA schedule runs in three phases:

How Well Does Spravato Work?
Spravato doesn't work for everyone, but for many people with treatment-resistant depression, results show up faster than with a new pill.
- In the study behind the 2025 approval, 22.5% of people taking Spravato alone reached remission by week 4, compared with 7.6% on placebo.
- Some people in the same study felt better within 24 hours of the first dose.
- Earlier studies paired Spravato with a new oral antidepressant and showed bigger gains than the antidepressant alone.
Remission means symptoms dropped low enough to count as minimal. Many more people improve without reaching full remission. Your prescriber tracks your progress with a short questionnaire at visits.
Spravato works best as part of a plan. Keep up therapy, sleep, and the basics while you're in treatment.

Most side effects show up during the visit and fade within a few hours. This is why you stay at least 2 hours.

Important Safety Information
Spravato carries an FDA boxed warning, the strongest kind, for these risks:
- Sedation, dissociation, and slowed breathing. You're watched for at least 2 hours after each dose.
- Misuse and abuse. Spravato is a controlled substance and stays at the certified office.
- Suicidal thoughts in young people. Antidepressants raise this risk in people under 25. Tell your prescriber right away about new or worse thoughts of suicide.

What are the side effects of Spravato?
The most common are feeling disconnected or dreamy, dizziness, nausea, sleepiness, a spinning feeling, and higher blood pressure. Most fade within a few hours, during or shortly after the monitored visit.
Can I take Spravato without an antidepressant?
Yes, for treatment-resistant depression. Since January 2025, the FDA has approved Spravato on its own. For depression with suicidal thoughts, Spravato is still given alongside an oral antidepressant.
Do I need a referral for Spravato?
Not always. A psychiatrist or psychiatric nurse practitioner evaluates you and prescribes Spravato. Some insurance plans want a referral from your primary care provider, so check your plan.
Who should not take Spravato?
People with an aneurysm, an arteriovenous malformation, a past brain bleed, or an allergy to ketamine or esketamine. Spravato isn't recommended during pregnancy and isn't approved for anyone under 18.
